Our office maintains birth and death records for persons
who were born or who died in Defiance County from December 1908 to present.

In office - need to complete a
request form and provide payment. Monday through Friday, 8:30 a.m. -
4:15 p.m. We accept cash, checks with a valid I.D., or debit cards (an
additional $2.50 fee is applied to debit card transactions). Birth/Death
records are usually issued within a few minutes. |

By mail - download a request form
for either a
birth or
death
certificate, complete the form and mail to the Defiance County General
Health District along with a check or money order made payable to
Defiance County General Health District, and a self-addressed, stamped
business-size envelope. Mailed requests are usually processed the same
