Defiance
County Health Needs Assessment Survey 2005
In the
fall and winter of 2005, an adult and a youth health needs assessment survey
was completed in the county. Coordinating this activity was Defiance
County Health Partners – led by staff of the Defiance County Health
Department. The adult survey was distributed within the community,
while the youth survey work was conducted in all county high schools as well
as Four County Career
Center. This report encompasses health beliefs, perceptions and practices of
residents of Defiance County.

Hispanic/Latino Community
Needs Assessment Survey 2003
In the fall of 2003, a community needs assessment survey was conducted
within the Defiance County Hispanic/Latino community. Work was coordinated
by the Tobacco Free Coalition of Northwest Ohio with guidance from public
health consultant, Kathy Snow MPH, RN. The Defiance County Health
Department was a member of this tobacco free coalition and supported this
needs assessment work. Survey work was completed at cultural festivals,
religious settings, community events and neighborhoods. Funding was
provided by the Ohio Tobacco Use Prevention and Control Foundation through
Women & Family Services, Inc. to identify health beliefs, perceptions and
practices of the Hispanic community regarding tobacco use and other health
